STEP 2: ENROLLMENT
Acceptance Letter
You will receive an email notifying you of your acceptance as a Sandia View Academy student.
Medical Insurance Card
Email a medical insurance card (front & back) to information@sandiaviewacademy.org
Health Record
Complete & submit the following forms:
Sports Physical
Code of Conduct
Voluntary Activities Participation Form
Immunization Record or Waiver
Email an Immunization Record or Waiver to: information@sandiaviewacademy.org
Current records must be on file at the beginning of each school year.
Identification
Email a Birth Certificate or Passport to Treasurer/Registrar Ruth Soto at information@sandiaviewacademy.org
